When to Use

  • When a bodily sensation brings up a frightening interpretation.
  • When you want to describe a symptom before discussing it with a clinician.
  • When health worries keep drawing your attention to what you cannot yet know.

How to Use

  1. 1
Describe where the sensation is, what it feels like and when it began; mark its strength.
  2. 2
Write the feared story and mark how true it feels right now.
  3. 3
Separate observed or professionally communicated information from guesses and unknowns.
  4. 4
Choose an appropriate next step or write your own, including medical advice when needed.

A physical sensation and a frightening interpretation can arrive together. Writing them separately can make space to describe what is happening without assuming what caused it.

The page asks for the sensation and its strength, the feared story and how true it feels, then what is known and unknown. A chosen next step can include seeking clinical advice; uncertainty is not proof that a symptom is safe.

Optional and private. This sheet is not medical advice and cannot determine whether a symptom is serious. Contact a health professional about new or worrying symptoms. For emergency symptoms or immediate danger, contact local emergency services.
